THE ALBUQUERQUE HEADING HOME PROJECT - The purpose of this project is to expand dedicated treatment and supportive services capacity to house Albuquerque's 75 most vulnerable, chronically homeless people who are struggling with substance use and co-occurring disorders. Funds will also be used to strengthen and formalize partner commitment to Albuquerque Heading Home, through consortium, dedicated housing resources, and leveraged local funds.This grant will accelerate formation of the inter-sector structure for leveraging resources and integrating systems, will result in measurable reduction of cost in public dollars for use of the emergency response and acute care systems, and - most importantly -- will increase service and system capacity to place the most vulnerable homeless people immediately into housing. Performance assessment and opportunities for comparative evaluation of the proposed mix of EBPs with peer projects nationally will direct the future of Albuquerque Heading Home and its sustainability.
