The Santa Clara County Department of Alcohol & Drug Services (DADS) proposes to provide intensive case management, treatment, and wraparound services to persistently homeless adults with active Drug Court cases. DADS will serve 140 (28 per year) individuals per year referred from Drug Court who are persistently homeless and have a substance abuse disorder diagnosis. It is anticipated that more than 63% of clients will be male, approximately one-third will be Hispanic/Latino, one-third White, with African Americans being disproportionately represented.
