BLUEPRINTS TO LIFE 2.0 - Community Bridges proposes to provide the Peer Support Recovery Program (PSRP to homeless individuals in Phoenix Arizona. Peer Support Specialists will provide outreach, engagement, support through treatment and assistance with accessing other support services. Peer Support Specialists will be primary in reengaging individuals in recovery support services if relapse occurs. The target population is men between the ages of 31-59. It is estimated that 73% will be White, 12% Native American, 13% Latino, 10% African American, and 2% Asian. Individuals that participate in the PSRP will establish a trusting, healthy relationship with a Peer Support Specialist who will: assist in the engagement in addiction treatment where necessary; support ongoing participation in treatment; assist in accessing healthcare, housing, employment; provide encouragement and support; and develop a realistic, achievable, and comprehensive relapse prevention plan.
