(LCCHC) began providing care to the Lafourche, Terrebonne, St. Martin, and St. Mary parishes in Louisiana in 2019 and has continuously, expanded health care services to include OB/GYN, dental, behavioral health, and primary care services. Additionally, with the growing patient demand and expanded service offerings, LCCHC opened another clinic in Houma, Louisiana and has recently become certified as an FQHC Look-alike. LCCHC is a trusted health care partner among the rural Louisiana community, supporting all patients, no matter socioeconomic status, gender, age, or ethnicity. The organization is committed to exceeding the expectations of all patients and providing patient-centric services. LCCHC provides Primary Care (risk screening, prevention, wellness care, chronic disease management, geriatric care, vaccinations, acute sick visits, children, and more), Family Behavioral Health (depression, anxiety, and stress reduction, including group, individual and family therapy and medication management), Substance Use Disorder Services, Laboratory Services, Screenings (TB), Diagnostic Radiology Services, Family Planning (contraceptive options, counseling), Dental Services, Case Management, Pharmaceutical Services, Transportation, and Translation Services and Immunizations (children, adult, COVID-19). The program will be carried out through planning efforts to implement an Internal Medicine Medical Residency Program by July 1, 2026 ,with a partnership between Life Coast Community Health Center, South Louisiana Medical Associates (SLMA), R3 Education and Thibodaux Regional Medical Center. The specific objectives of this program include: • LCCHC Medical Residency program developed and ready for implementation by July 1, 2026 • Minimum of 5 Medical Residents complete the LCCHC Medical Residency program annually. • LCCHC Medical Resident participants are successful in the program annually. • All LCCHC Medical Resident participants gain full time employment as upon completion of program and remain in the service area. • Sustainability plan for Medical Resident program developed. Residents who participate in the Internal Medicine Program will complete rotations across LCCHC (at least 50% of their time), Thibodaux Regional Medical Center, and SLMA for ACGME required core and elective courses and didactic trainings. Life Coast Community Health Center proposes to have 5 Internal Medicine Medical Residents placed in Residency Programs each year. The MD Residency start date will be July 1, 2026. There will be a new class of MD residents annually each year in July. The goal will be to train 5 Medical Residents through this program and place each of these Residents in full-time employment clinical sites that are rural and/or underserved in South Louisiana.
