May, MBA Proposed Residency Type: Family Medicine Residency Program Funding Preference Statement: YES – AHEC Partnership Population Target Area(s): Morrisville, Vermont Funding Amount Requested: $500,000 Projected Number of Residents: 30 residents; 10-10-10 Anticipated ACGME Accreditation Date: April 2024 Anticipated Residency Matriculation Date: July 1, 2025 Lamoille Health Partners is a Federally Qualified Healthcare Center incorporated as a 501c(3) nonprofit organization in 1998 and was part of Copley Health Systems; a network that included a critical access hospital. Subsequently, LHP became an independent nonprofit organization in 2005. LHP became a FQHC to serve the rural population based on the needs of the residents of Lamoille County. LHP serves Lamoille County, including the following communities and villages: Belvidere, Cambridge, Eden, Eden Mills, Elmore, Hyde Park, Jeffersonville, Johnson, Morristown, Waterville, Wolcott, and Stowe; with high-quality low-cost comprehensive medical services for all life cycles, from pediatrics to geriatric care. Services are primarily located in Morrisville with a family practice site in Stowe. There are no other comprehensive health care services easily available to Lamoille County residents without a significant commute. LHP will Obtain ACGME accreditation for a 10-10-10 Family Medicine residency program; Matriculate the initial class of Family Medicine residents; and identify and implement strategies and funding streams to financially sustain, and potentially expand, the residency program. The proposed family medicine residency will be specifically tailored to meet the needs of those disadvantaged communities and residents who qualify as rural, underserved, under-resourced, and uninsured throughout the state of Vermont by creating GME Consortium comprised of 6 FQHCs, their partnering hospitals, and academic institutions. The proposed GME Consortium will be composed of FQHCs in Vermont, their partner hospitals, Bi-State PCA, UVM, Dartmouth, and community stakeholders. Per the requirements of the THCGME funding and initial legislation from 2010, the FQHCs will hold a significant role in the academic, administrative, and financial functions of the consortium and the residency program. LHP plans to be strategic in developing curricula and relationships with participating sites to best meet the previously stated needs of the community. To address the burden of the aging population and manage the chronic diseases they face, LHP will participate in community outreach activities focusing on the populations that include the elderly, homeless, seasonal workers, and adults and youth with substance abuse and opioid use disorders. To address the physician shortage in this rural area LHP will work with academic partners in the area who also have a rural focus, strategically placing themselves in a position to be a champion and leader in rural health care.
