Burleson, Grimes, Leon, Madison, Milam, and Robertson Counties are deemed rural by the Federal Office of Rural Health Policy and are HRSA-designated Medically Underserved Areas (MUA) and Primary Care Health Professional Shortage Areas (HPSA). Brazos County, where the program will be administered, is a non-rural HRSA-designated Primary Care HPSA and MUA (9 census tracts). The target population includes the 119,224 residents of HealthPoint’s rural service area. 23% (27,355) of our service area are children under 18-years-old, and 19% (23,027) are ages 65 years and older. The area deprivation score and percentile are 108.4 and 69, respectively, indicating higher levels of deprivation, which are associated with higher risks of preventable health conditions. 15% of people in our service area are living in poverty (below 100% federal poverty level). 34.6% of the population has public insurance, while 14.6% do not have insurance. 18% of the total population and 25% of children are food insecure. 64% of children are eligible for free and reduced price lunch, which is higher than the U.S. average of 49%. 31% of adults are obese, and 12% of adults have diabetes. The leading causes of death are cardiovascular disease and cancer and other tumors with higher age-adjusted mortality rates than state and national for cardiovascular disease, diabetes, infections, and deaths from cirrhosis and chronic liver disease. Brazos Valley Community Action Agency, Inc., (dba HealthPoint)—a federally qualified health center, as defined in section 1905(l)(2)(B) of the Social Security Act [42 U.S.C. 1396d(l)(2)(B)—is applying on behalf of a Graduate Medical Education (GME) Consortium consisting of HealthPoint, Sam Houston State University College of Osteopathic Medicine (SHSU-COM), and its affiliated hospital and clinic partners for funding under the Teaching Health Center (THC) Planning and Development Program to establish a new ACGME-accredited, community-based graduate medical residency training program in family medicine. Consortium members will form a Graduate Medical Education Committee consisting of representation from all partner organizations. SHSU-COM will provide THC planning and development technical assistance throughout the institutional and program accreditation process. HealthPoint Navasota will serve as the Continuity Clinic, and HealthPoint clinics in Brazos County and other surrounding rural areas will serve as additional core and elective outpatient rotation sites. SHSU-COM serve as the Sponsoring Institution and academic partner, and its hospital affiliates will serve as inpatient rotation sites. The goals of this project are to • achieve ACGME Institutional accreditation, • achieve ACGME Program accreditation, • ensure sustainability of the program beyond the period of performance, and • track residents’ career outcomes for a period of at least one year post-graduation from the residency program.
