IBHS began in 1976 as a substance abuse treatment center, and began providing primary care services in the 1990’s. In 2002, IBHS became a federally-funded Community Health Clinic (CHC) and is committed to providing excellent service in the areas of physical health care, substance abuse treatment, mental health improvement, homeless services, and prevention education at four locations: three in the City of San Bernardino in San Bernardino County, CA and one in the City of Banning in neighboring Riverside County, CA. Services at these sites address major health care needs, barriers to access, and social determinants of health outcome disparities in the service area, including disproportionate rates of cardiovascular disease, hypertension, diabetes, asthma, poor perinatal indicators, oral health, and behavioral health care. IBHS is seeking to develop and obtain accreditation for a new [6-6-6] FMRP with a total of 18 clinicians who will receive high quality training and an approach to medicine that is compassionate, culturally sensitive, and equitable in meeting the unique primary care needs of the underserved populations in IBHS’s service area (homeless, low-income, LGBTQIA, migrant workers, minority groups, moderate-to-severe mental health, public housing recipients, substance abuse, women & children, and seniors). IBHS’s patient population is medically and socially complex and increased access to primary care by physicians who are culturally and linguistically sensitive will be key to improving patient and population health outcomes in its service area. To ensure that IBHS’s FMRP develops providers attuned to the needs of their community, IBHS will leverage its strong community partnerships. This includes expanding its existing relationship with Dignity Health Community Hospital of San Bernardino (DHCHSB), IBHS’s formal partner in the FRMP. Aligned with IBHS’s mission, DHCHSB is committed to increasing the diversity and distribution of health professions students and practitioners in underserved communities in the Inland Empire in California. By becoming a Sponsoring Institution and developing a new CHC-based FMRP in a region that already has the lowest physician to resident ratio in California, IBHS will expand the physician pipeline in its service area with linguistically and culturally responsive clinicians, increasing access to care for underserved communities impacted by social determinants of health and ultimately reducing health outcome disparities in the special populations IBHS serves. It will also further IBHS’s work in team-based care to address the “whole patient” and population health in the service area.
