The Work Plan is aligned with the four CHWTP goals (1. Expansion, 2. Extension/Upskilling, 3. Employment, 4. Equity) and has an SDOH-oriented methodology with three strategies (1. Transformative Learning; 2. Dynamic Partnerships; and 3. Lifelong Learning.) In three years (September 15, 2022 to September 14, 2025), we propose to serve 150 participants, at least 50% of whom will serve in Health Provider Shortage Areas and represent underserved populations, particularly ethnic minorities. Trainees, CHWs, health support workers and apprentices will receive training and employment opportunities, as well as financial support, consistent with HRSA workforce program aims: access, supply, distribution, and quality. Three apprentice cohorts (28 apprentices total) will participate in experiential learning in employer-sponsored apprenticeship placements across a wide, cross-sector range of entities.
