Catholic Family Center (CFC) project will create a seamless and stable network of behavioral health treatment, primary health care and health (wellness) promotion services that will improve health outcomes for individuals with severe mental illness who may also have co-occurring substance use disorders. The coordinated physical and behavioral health care which CFC is proposing to implement, in tandem with intensive Health Home care coordination and case management, will result in the following for program participants with SMI: an increase in stable primary health care, a decrease in hospital admissions and readmissions for the individuals we serve, a decrease in unnecessary ED visits, and an increase in overall health. These achievements will also result in a decrease in the per capita cost of care. Our project goals are centered on three pillars of wellness: Provider integration, Wellness promotion, and Recovery Support = PWR. Our agency is well-positioned and qualified as our City's largest human services provider; providing direct services to 34,000 in 2014. CFC's mental health services included an outpatient clinic and satellite clinic for those with co-occurring mental health and substance abuse disorders. Our project, the PWR Health Home Program will serve a diverse population of at least 600 individuals during a four-year project period (60 unduplicated individuals in Y1, 150/Y2, 240/Y3, 300/Y4) with the following goals: Provider Integration: Improve physical health status via a co-located network of behavioral health treatment and primary care providers. Wellness Promotion: Implement healthy lifestyle behaviors through life skills education, nutrition, physical activity, and smoking cessation programs. Recovery Support: Improve quality, access of care by addressing client needs with comprehensive care management and care coordination.
