Juneau Primary and Behavioral Health Care Integration Project (JPBHCI). The overall goal of the JPBHCI project is to improve the health status of adults with SMI and those who have or are at risk of having co-occurring substance use disorders, who have ex- perienced trauma, and with or at risk for chronic medical conditions. This will be accomplished by integrating primary care, within a behavioral health setting and providing services that are coordinated, cost-effective, person-centered and take a holistic approach to wellness. The proposed JPBHCI project includes a partnership between Juneau Alliance for Mental Health, Inc. (JAMHI) and the Front Street Community Health Center (FSCHC), a FQHC located in Juneau, Alaska. The target population for the project is all adults in the City and Borough of Juneau service area over the age of 18 who have SMI and are without adequate primary care services. The priority population for this project is adults with SMI and ooccurring substance use disorders and who have a history of trauma. JAMHI anticipates serving 31 individuals in Year 1 and 178 unduplicated clients throughout the course of the project.
