The Monadnock Regional System of Care Collaborative - The opportunity of this Planning Grant will benefit the entire Monadnock Region which includes the County of Cheshire and its' surrounding communities with a population of 101,401 citizens and align with the State of NH Implementation Plan for Children's Mental Health. We will accomplish this goal by 1) Creating a diverse regional children's behavioral health collaborative through a shared visioning process, 2) Developing an asset map of current services and a gap management plan for the expansion of a System of Care in the Monadnock Region, 3) Developing a workforce plan to create an integrated System of Care evidence-based framework, 4) Engaging children and youth with serious emotional disturbances and their families within the Monadnock Region in the development and expansion of the Systems of Care, 5) Analyzing state and system policies and strategic plans around mental health issues, 6) Developing a plan to educate policy makers, relevant governmental entities, multi-disciplinary stakeholders, higher education, payers and the provider community within the Monadnock Region, 7) Evaluating ways to share data and outcome measurement across multiple systems of care to avoid duplication of services and gaps of knowledge, in order to employ solutions to better collaboration and integration of children and youth serving organizations and 8) Fostering relationships with the current Managed Care Organizations in NH to help coordinate mechanisms to integrate the SOC approach, including optimizing and coordinating all payer resources that meet the needs of providers who serve children, youth and families in the Monadnock Region.
