Appalachian Integrated Care & Wellness Program - Highland Rivers Health in partnership with Georgia Mountains Health are submitting a proposal to establish an integrated care clinic and an integrated mobile team to treat the co-morbid medical and behavioral needs of citizens living in the Appalachian region of North Georgia. The project thereafter referred to as the Appalachian Integrated Care and Wellness Program; will be a vital access point to care for the 170,000 plus low income citizens currently residing in the four county regions of Whitfield, Murray, Fannin and Gilmer Counties. The goal of the partnership will be to provide local, evidence-based and culturally competent integrated care services for low income individuals that have significant barriers to care due to geographic and transportation barriers that are inherent to a rural mountainous community. The Appalachian Integrated Care and Wellness Program will serve a minimum of 200 individuals in Year 1, 375 individuals in Year 2, 475 individuals in Year 3 and 600 individuals in Year 4. The program partners have engaged Dr. Ben Druss M.D., from the Emory Rollins School of Public Health, to naturally expand his current evaluation of a current SAMHSA grantee and NIMH research study to focus on a specific sub-population of disenfranchised individuals who have a serious mental illness.
