COMPREHENSIVE HEALTH HOME FOR THE UNDERSERVED MENTALLY ILL - Family & Children's Services (FCS), a Community Mental Health Center (CMHC) in Tulsa OK, in partnership with Morton Comprehensive Health Services (MCHS) ), a Federally Qualified Health Center (FQHC), will establish comprehensive health home services to improve the physical health status of consumers who have both Serious Mental Illness (SMI) and one or more chronic health conditions. To improve health outcomes, the Comprehensive Health Home for the Underserved Mentally Ill (CHHUMI) will build a person-centered system of well coordinated care and patient activation through integrating the advanced mental health and addiction treatment practices of FCS with comprehensive primary care services provided by MCHS within a collaborative health home environment onsite within the CMHC. Specific goals: (1) Integrate comprehensive primary and specialty medical services with behavioral health services in a community mental health setting-Objectives include increasing screening, prevention services and consumer engagement; (2) Improve the health status of SMI consumers with chronic disease through use of evidence based care management practices- Objectives: increase disease management for cardiovascular disease, Asthma/COPD, Diabetes and Medication adherence; (3) Improve quality, access and reliability of care by facilitating transitions of care and providing enhanced care coordination across Medical neighborhood- Objectives: Reduce hospital re-admission, ER use and increase care transitions; (4) Improve patient empowerment and self management-Objective: increase self care and participation in wellness activities; (5) Integrate health information technology into the health home setting and the provider/care team level-Objective- Achieve Meaningful Use, Health Information Exchange use. The program will serve 250 SMI consumers in Year 1, 405 in Year 2, 525 in Year 3, and 630 in Year 4 - total of 1,810.
