Omaha Health Home Collaborative - Community Alliance Rehabilitation Services proposes the Omaha Health Home Collaborative (OHHC) to transform care delivery for adults with serious mental illness (SMI) who have, or are at risk for, co-occurring chronic health issues by integrating primary care services, in collaboration with One World Community Health Centers. We will establish on-site primary care services at Community Alliance and initiate comprehensive Health Home services by incorporating FQHC services within our agency; adjust our care management and coordination policies and integrate our electronic health record to facilitate collaboration among health and mental health providers. We will transform our community support care managers into Health Navigators and our existing cadre of peer support specialists into Peer Health Coaches. We will expand our array of evidence-based practices to support our integration plans and establish an integrated Wellness Plan to holistically coordinate behavioral health and health treatment, wellness promotion, and recovery support. We will utilize Wellness Self Management as a framework to promote client empowerment and self-management. We will formalize and enhance our array of health promotion activities, train our workforce to ensure culturally competent care, and promote health literacy. Our goals are to 1) Establish on-site primary care services to 200 clients in Year 1, 375 clients in Year 2, 475 clients in Year 3 and 600 clients in Year 4; 2) Provide holistic healthcare; 3) Offer an array of health workshops and wellness programs to address tobacco cessation, improved nutrition, promote health literacy, improve client self management and promote regular exercise; 4) Establish integrated care planning; 5) Strengthen program participants' role in the oversight of the project and ensure cultural values, norms and beliefs of program participants are respected and integrated into the treatment process; 6) Strengthen our quality assurance systems.
