Institute for Community Living's Healthy Living Program will transform 3 mental health treatment programs into integrated care settings with a comprehensive array of health home services with M H clinicians at the treatment team nexus. The 4-year grant will enable ICL to develop, implement and evaluate a Behavioral Health Home that will effectively deliver integrated care to low income adults with serious mental illness. Model components include 1) comprehensive assessments to facilitate medical risk stratification; 2) care managers (CM) helping to implement the treatment plan and coordinate care; (3) on-site primary care providers (PCP) offering accessible medical services; and (4) peer health coaches providing health education and individual support. Over the project life, ICL will serve 750 adults, 18 years and older, with SMI and 200 in year 1 at 3 New York State (NYS)-licensed M H treatment programs located in Brooklyn, New York. Two of these sites are mental health clinics, and the third is a clinical rehabilitation program. ICL will integrate the project with its Brooklyn NYS-designated HH (NYSHH) and the federally qualified health center (FQHC) it founded, Healthcare Choices (HCC). ICL will subcontract with HCC to co-locate PCPs at the project sites to facilitate primary care access for participants poorly served by the mainstream system, and provide specialty care at their Brooklyn clinic. Two additional FQHCs and 4 medical centers with multiple community clinics are also collaborating to provide specialty care. Health Information Technology will facilitate integrated care regardless of physician location. The Project's goals are to: (1) Complete organizational changes needed to fully integrate primary and specialty care services at the 3 project sites; (2) Complete transformation of workforce in each site to support integration of services; (3) Enhance consumers' experience of care; and (4) Reduce/control per capita cost of care for people with SMI.
