Springfield Health and Recovery Program (SHARP) - Behavioral Health Network (BHN) and Caring Health Center propose to create a collaborative health home program for adults with SMI, based on PCMH principles for integrated service delivery, which includes the Stanford chronic care model and medical management supported by prevention, wellness programming, advocacy and health education. Partners in SHARP include Caring Health Center to deliver integrated primary care, care management and wellness services, and the Western Massachusetts Recovery Learning Community (a consumer-operated organization), to provide peer-guided wellness groups and activities as well as Peer Specialists. Participants in SHARP will also have access to the full range of BHN behavioral health programming and Caring Health Center medical services. The program will serve 200 adults, 18 years old and above, and their families in the first year, growing to 600 clients over the four-year grant period. The unduplicated total over the grant period will be 600 people. Primary care services will be integrated with behavioral health services through the initiation of a multidisciplinary Treatment Team guided by individually developed Care Plans, comprised of SHARP behavioral health, primary care, care management and peer staff. Services will include screening, assessment, medical management, medication monitoring, advocacy and wellness services delivered through team care coordination by a Nurse Care Manger. A comprehensive array of wellness and health education programs will support the clients in making positive choices about their health and health care. The program is located at the BHN Liberty St. outpatient clinic site in Springfield. Individual client status and collective program status will be tracked through the BHN electronic medical record system.
