Building upon an award-winning local System of Care, BECOMING (Building Every Chance Of Making It Now and Grown-up) will enhance and expand the services and supports available to Durham's 16-21 year olds with serious mental health issues and their families. Uilizing Evidence Based Practices (EBPs), community, and natural resources BECOMING will encourage and support transition-age youth to plan for and negotiate a successful transition to adulthood. The goals of this project are to improve access to services and supports for high risk transition-age youth as well as improve the coordination and continuity of services up to and during this transition period. Specifically, BECOMING will 1. Bridge the child and adult service system chasm with a more effective and comprehensive approach, inclusive of all life domain areas, 2. Develop a system that is equipped to address high risk transition age youth's clinical, developmental, and social needs including educational attainment and workforce connections, and 3. Address service system and policy barriers with statewide dissemination.
