Healthcare Home Project Healthcare Information Technology Enhancement - The Healthcare Home Project Healthcare Information Technology Enhancement will work with Qualifacts, a SaaS vendor, to move from our current EHR platform, Qualifacts Carelogic v4, to a fully ONC Certified Record, Qualifacts Enterprise Platform. Milestone and two other PBHCI grantees, Prestera and CMHA, developed with Qualifacts a plan to enhance the functionality of inbound CCDs. Joining forces to contribute to the cost of the enhancement made possible an improved EHR which would have been otherwise unaffordable. The enhanced CCD functionality will change the way the CCD displays in the Enterprise Platform. The current functionality allows incoming CCDs that use the HL7 format to display in the consumer's record as a complete document in a module labeled "document library". This enhancement will split the CCD into its specific data elements and populate those elements into the specific modules in the EHR that contain those elements. Integrating the data from the PCP generated CCD into the specific modules where similar data resides ensures the information will be considered in making clinical decisions which will improve health outcomes. The Healthcare Information Technology Enhancement will not only benefit the 168 seriously mentally ill adults projected to be enrolled in the Healthcare Home Project during the year. It will also provide a meaningful use certified EHR to approximately 2,700 individuals receiving services at Milestone during the year.
