Using genetics in the selection of candidates for therapies to prevent type 1 diabetes and its progression. - Our long-term goal is to predict and prevent type 1 diabetes (T1D) and its progression after onset. The overall objective is to integrate genetics into the selection of individuals who will benefit from T1D disease-modifying therapies (DMTs) to prevent progression through preclinical stages of T1D and preserve beta-cell function after clinical diagnosis (stage 3 T1D). The central hypothesis of this application is that genetics, in combination with other factors, can identify individuals at risk for developing T1D and those likely to respond to DMTs. The rationale is that timely and accurate selection of individuals who will benefit from DMTs will improve the risk-benefit ratio. In the previous cycle of this grant, we genotyped 4,680 participants in TrialNet Pathway to Prevention (observational arm) and Prevention Trials using a genome-wide single nucleotide polymorphism (SNP) array. Then, we successfully incorporated a T1D genetic risk score (GRS, a combination of HLA and non-HLA SNPs associated with T1D) into T1D prediction models and developed a T1D risk predictor to facilitate implementation in clinical research and practice. We also tested the influence of the T1D GRS on response to prevention trials. Finally, we have successfully made this new resource available to investigators within and beyond TrialNet. In this proposal, guided by strong preliminary data, we will test our central hypothesis by two new specific aims: (1) Identify SNPs and/or trait- or pathway-specific polygenic scores (PGSs) that determine response to prevention and new-onset trials. Using available data from participants in prevention or new-onset trials conducted by TrialNet or the Immune Tolerance Network, we will analyze the influence of SNPs involved in biological pathways targeted by specific DMTs or associated with immunologic signatures of response, such as the T cell exhaustion pathway. (2) Refine T1D risk prediction using SNPs and trait- or pathway-specific PGSs to understand T1D heterogeneity. We propose to study TrialNet Pathway to Prevention participants to address important gaps in knowledge that remain in T1D prediction, such as the optimal use of genetics in understudied subsets of individuals, including those with atypical characteristics or from non-European ancestry or who are adults. his project is significant because it will enable precision selection of candidates for DMTs, thereby improving prevention and preservation of beta-cell function after diagnosis. This project is innovative because it seeks to shift the current practice by proposing to utilize genetics as a novel, affordable, time-independent strategy to identify individuals likely to respond to intervention.
