Developing a multidisciplinary telemedicine model to address gaps in chronic pediatric uveitis care - Abstract Chronic pediatric uveitis is a rare, debilitating eye condition that can severely affect both the eye health and overall quality of life for children. Without proper management, this condition can lead to serious complications such as cataracts, glaucoma, retinal damage, and even blindness. Due to the complex nature of the disease, it requires care from pediatric ophthalmologists, pediatric rheumatologists, and other specialists, often involving long-term monitoring and treatment. However, diagnosing and managing chronic pediatric uveitis is particularly challenging for children, as many are unable to communicate symptoms, leading to delays in diagnosis and treatment. Furthermore, families face substantial challenges in obtaining care, particularly in remote areas, where issues such as geographic location, income, health literacy, and limited healthcare resources exacerbate difficulties. The shortage of uveitis specialists often forces families to travel long distances, delaying timely diagnosis and care. To address these challenges, we propose a telemedicine-based care model supported by a multidisciplinary team that offers remote care from pediatric ophthalmologists, uveitis specialists, and pediatric rheumatologists. This model will reduce the need for travel and allow for improved monitoring, while local technicians can collect health data and imaging. In addition, virtual collaboration with social workers, psychologists, and parent support groups will address the psychosocial struggles faced by children and their families. Successful development of this telemedicine model will depend on strong community partnerships, facilitated by the Community Advisory Group we have established. This group brings together a wide range of perspectives from caregivers, healthcare providers, social workers, advocacy organizations, professional societies, and community groups to ensure a comprehensive approach. Specifically, our Aims are to (1) identify barriers and facilitators in pediatric uveitis care through focus groups with key stakeholders and community partners; (2) develop a multidisciplinary, team-based telemedicine care model to address gaps in pediatric uveitis diagnosis and management through Community Engagement Studios; (3) test the feasibility of the telemedicine care model. By leveraging innovative telemedicine solutions and community-driven collaboration, this project aims to expand care, ensuring that every child with chronic uveitis receives timely, effective, and compassionate treatment, regardless of their location, financial means, or background.
