Unity Health Care, the primary medical provider for the District of Columbia's Department of Corrections, proposes an innovative and comprehensive program to enhance care transitions for justice-involved individuals (JI-R) reentering the community. This project aims to bridge critical gaps in health care and social services that disproportionately affect JI-R individuals, who often suffer from chronic conditions, mental health disorders, substance use disorders (SUDs), and infectious diseases. Recognizing the profound barriers to successful reentry, our program will implement a robust, integrated care model that includes pre-release planning, intensive care coordination, and holistic support services for patients with only 90 days left to reenter the community. By fostering strong community partnerships and utilizing evidence-based practices, we aim to ensure continuity of care, improve health outcomes, and support the reintegration of JI-R individuals into society. This initiative will not only address immediate health needs but also tackle social determinants of health, ultimately promoting long-term stability and reducing recidivism. It would also pair individuals with a peer-support navigator, helping them navigate the health care and community systems to better manage their needs. Unity Health Care is committed to transforming the health landscape for justice-involved populations and creating a sustainable model of care that can be replicated and scaled across similar settings.
