Packard Health cares for patients at every stage of life, believing that everyone deserves equitable access to health care. Packard Health offers an array of health care services to its patients including primary care for the whole family, women’s health, prenatal care, behavioral health, chronic disease management, addiction treatment services, telehealth, community outreach services, insurance enrollment assistance and transportation assistance and translation services. Its service area consists of seven (7) zip codes located in Washtenaw County, which area experiences significant disparities in employment rates, poverty levels, and educational attainment–all neighborhood risk factors that may increase likelihood of reincarceration post-release. To mitigate these risk factors, Packard health is working to develop a partnership with Washtenaw County Corrections to provide requested Medication Assisted Treatment services to individuals residing in the facility. Through QIF-TJI funding will also provide care coordination and planning services for individuals within 90 days of release to ensure prompt connections with healthcare providers and providers of health-related social services. Critical Health Needs to be addressed through programming include reducing risk of overdose and addressing mental health and substance use disorders treatment needs. Packard Health will also facilitate case management services and collaborate with community partners to address Health Related Social Needs, including housing insecurity, food insecurity, financial strain, lack of transportation, and other needs as identified. To adequately address individual needs, Packard Health will partner with Washtenaw County Sheriff’s Office to coordinate transitions in care to community-based providers upon reentry. Packard Health will also establish referral processes with community-based providers, including Avalon Housing, Housing Access for Washtenaw County, Food Gatherers, and Feonix Mobility to meet individuals’ health-related social needs. Other program activities will include the provision of: - Health evaluation(s), to identify acute and chronic physical health, behavioral health, or oral health issues and associated needs. - Primary health services including diagnosis, treatment, and medication management. - Immunizations. - Behavioral health services, including MAT/MOUD and peer recovery support. - Case management services connecting J-IR individuals with federal, state, and local benefits programs to address critical health and health-related needs. Through HRSA QIF-TJI funding, Packard Health will increase the number of JI-R individuals who: - Receive wraparound support from community-organizations and post-incarceration. - Receive in-scope health center services post incarceration. - Are enrolled in eligible local, state, and federal benefits programs post-incarceration.
