Quality Improvement Fund - Justice Involved - In the Ohio counties of Erie, Huron, and Ottawa, justice-involved adults face significant challenges reintegrating into society, contributing to a 30% recidivism rate. The Transitions in Care for Erie, Huron, and Ottawa County initiative aims to disrupt this cycle by providing comprehensive support. The Erie County Health Department/Erie County Community Health Center will lead a tri-county collaboration with the Erie, Huron, and Ottawa County Jails, and key stakeholders to establish a "Circle of Care" for justice-involved adults transitioning back into the community. This innovative approach utilizes the Coordinated Care Model, Trauma Informed and Trauma Responsive Care Models, and the Wraparound Process to deliver integrated services. Key Activities: • Bio-psycho-social screenings within 30 days of release. • Case management with individualized support plans for up to one year post-release. • Access to healthcare, SUD treatment, housing, social services, and job training. • Evidence-based interventions to address underlying factors contributing to justice involvement. The goals of this program are to: 1) improve follow-up and completion of clinical services to achieve superior outcomes once individuals return to their home/community; 2) to reduce recidivism rates among adult populations; 3) to enhance the capacity of the justice system to meet individual needs; 4) to promote healthier lifestyles and positive development for jusice-involved adults and their families, including children. Expected outcomes of the project include improved healthcare access and reduced healthcare disparities for justice-involved adults and their families, reduced recidivism rates, enhanced competency skills and character development among justice-involved adults, and increased engagement with community resources and supports. The primary service area for this initiative encompasses Erie, Huron, and Ottawa Counties in Ohio. The intended beneficiaries of this project are justice-involved adults, their families, and the broader community. By providing wraparound support and addressing the root causes of justice involvement, this initiative aims to break the cycle of incarceration, promote healthier lifestyles and positive development for justice-involved adults, and contribute to safer and healthier communities in the region.
