ABSTRACT The state of Hawaii is spread across a 1,500-mile non-contiguous archipelago, with 34% of the population living in rural communities, Health promotion programs in Hawaii, including cancer prevention services, are challenging within these rural and medically underserved communities, who have limited access to healthcare. Through the support of an Administrative Supplement to the Cancer Center Support Grant, the University of Hawaii Cancer Center (UHCC) has utilized a Community Health Educator (CHE) as part of the UHCC Community Outreach and Engagement (COE) component to extend our evidence-based, community engagement and education strategies to reach these rural and medically underserved communities. The long-term goal of these activities is to increase knowledge and access to cancer prevention programs and reduce the local burden of cancer. In March 2020, statewide COVID-19 health and safety protocols were enacted in Hawaii, limiting travel and public group interactions, curtailing our CHE’s outreach efforts. However, our CHE successfully adapted the in-person sessions on clinical trials and HPV vaccination to online sessions using Zoom and Facebook Messenger. This proposal intends to extend and enhance these effective community outreach strategies used during COVID-19 restrictions. We will add the support of a Research Assistant (RA) to help conduct both on-line and in-person community-based group education and discussion sessions. We will continue to promote participation in clinical trials, knowledge of and uptake of HPV vaccination, and expand our work towards increasing colorectal screening through the Screen to Save program. We will use either on-line or in-person education depending on state and county COVID-19 health and safety precautions. We expect to reach 400 participants statewide annually during two years. Our Specific Aims are to: 1) Work with UHCC stakeholder and advisory groups using community-based participatory approaches to continue to culturally-adapt clinical trial, HPV vaccination, and Screen to Save (S2S) educational resources from the NCI’s National Outreach Network (NON) and locally developed resources. These resources will continue to be used with Hawaii’s medically underserved rural, Native Hawaiian, and Pacific Islanders populations; and 2) In collaboration with UHCC’s community cancer control partners, the Hawaii Comprehensive Cancer Control Program, the Hawaii Primary Care Association, and the Hawaii Public Housing Authority, our CHE will reach a total of 400 Native Hawaiian, Pacific Islanders and rural participants to conduct community-based group education and discussion sessions. The sessions will be held as permitted within COVID-19 guidelines as either in-person or on-line sessions with ~8 to 20 participants each, on clinical trials, HPV vaccinations and colorectal cancer screening. We expect to hold ~20 to 40 groups annually, over two years
