Grant #T0BHP28582 ABSTRACT BODY CONTENT: Overview/How the project will be accomplished: FMRWM will plan and develop Rural Track Programs (RTPs) in rural Butte and Helena, Montana. The Butte and Helena RTPs will augment the primary care workforce serving rural and medically underserved populations in the State of Montana by: 1) establishing new ACGME accredited RTPs at the two new sites; 2) developing and implementing a sustainability plan which includes ongoing funding streams to sustain long-term resident training; and 3) implementing a graduate tracking plan for residents participating in the new RTPs. Clinical collaborators/consortium members for the project include Providence St. Patrick Hospital, Community Medical Center, Partnership Health Center, St. Peters Health, PureView Health Center, St. James Hospital, and Southwest Montana Community Health Center. Measurable objectives: Objective 1: Establish two (2) new ACGME accredited rural track programs in family medicine with enhanced obstetrical training. Objective 2: Create RTP programs that qualify for Medicare and Medicaid GME payments. A long-term sustainability plan will fund resident training through: 1) Medicare GME payments (IME and DGME), and 2) Medicaid GME payments. Objective 3: Develop one (1) structured graduate tracking plan to track and publicly report resident career outcomes post-graduation for a period of at least five years after the first graduating class for the purpose of determining retention in rural communities. Expected Outcomes: 2 new RTPs in family medicine with enhanced obstetrical training established; ACGME application submitted for each new RTP; achieve ACGME accreditation for each new RTP; 1 financial sustainability plan developed; 2 new RTPs qualifying for Medicare and Medicaid GME payments; 1 graduate tracking plan implemented for the new RTPs; initial residents matriculated by July 2027 (Helena) and July 2028 (Butte); ultimate resident compliment increase of at least 3 residents per class or 9 total.
