Project Description: The Oswego County Integrated Delivery Network (OCIDN) seeks funding under the HRSA Rural Health Network Development Planning Program to enhance collaboration among local health care providers, community-based organizations, and social service agencies. The proposed project focuses on strengthening the rural health care system in Oswego County by fostering integrated partnerships, enhancing care coordination, and improving access to essential services for underserved populations. Needs Addressed: Oswego County faces significant rural health challenges, including provider shortages, fragmented care delivery, and disparities in access to preventive and specialty health services. The region experiences high rates of chronic disease, behavioral health concerns, and socioeconomic barriers that impede residents' ability to receive timely, coordinated care. This project aims to address these gaps by establishing a sustainable network infrastructure that supports information sharing, cross-sector collaboration, and resource alignment to improve health outcomes. Proposed Services: • Complete strategic planning to identify priorities that align with current needs and the local healthcare landscape • Build on lessons learned to enhance and expand integrated models that connect medical, behavioral health, and social services more effectively. • Implement a comprehensive data-sharing platform to enable real-time information exchange and enhance communication among network partners. • Identify funding and policy strategies to support long-term network viability.
