The Utah Center for Rural Health at Southern Utah University (UCRH-SUU)’s focus area is improved efficiency in care delivery. Proposed Service Region. - Coalville, UT (Summit) - Kamas, UT (Summit) - Duchesne, UT (Duchesne) - Roosevelt, UT (Duchesne) - Richfield, UT (Sevier) - Enterprise, UT (Washington, Rural Census Tract: 270200) Population to be Served. This proposal will serve rural residents of the proposed service region primarily below the low-income and the elderly, due to the Rural Health Clinic's focus on Medicare and Medicaid populations. The proposed service region is home to high populations of indigenous peoples, as well as increasing numbers of Asian Americans, members of the LGBTQ+ community, and immigrant/migrant workers. All of these populations will see improved access under the proposed network. Network Members. - Coalville Clinic (Rural Health Clinic) - Kamas Clinic (Rural Health Clinic) - Main Street Clinic (Rural Health Clinic) - Mountain Utah Family Medicine (Rural Health Clinic) - Uintah Basin Medical Center (Rural Health Clinic) - Utah Center for Rural Health/Southern Utah University (Institution of Higher Education) - Utah Office of Primary Care and Rural Health (State Office of Rural Health) - The Rural 9 Network (Non-Profit Cooperative Association) Of the five network partners who provided letters four qualify as rural, and one does not, meeting the HRSA requirement of sixty-six (66%) of the network members physically located in rural communities. Each of these potential network partners is fully ready and committed to integrating their functions into the proposed Utah RHC Network. Experience in Serving Rural Underserved Populations. As Utah’s State Rural Health Association, UCRH-SUU has an extensive history of serving rural populations. UCRH-SUU has been awarded and carried out four HRSA grants, including an RCORP Planning Grant, two RCORP Implementation Grants, and an RCORP-Psychostimulant Grant. UCRH-SUU has also managed eight state grants and a foundation grant over the past five years, each focused on bringing community partners across rural Utah together to address various health concerns. UCRH-SUU has also been active in assisting the State Office of Rural Health in conducting needs assessments and planning, including multiple projects in response to COVID-19. Under this proposed project, UCRH-SUU will work to improve efficiency and outcomes among the four Rural Health Clinic Partners and plan to grow that impact across new network members upon completing the grant work plan. Facilitating collaboration, will reduce wait times, and improve outcomes for rural residents in these areas across the continuum of care. Most importantly, UCRH-SUU seeks to ensure that no RHC in the state of Utah faces potential closures that would leave significant and harmful gaps in the availability of care for these populations.
