Proposed Service Region: Proposed service region includes the following rural counties: Sioux County, Iowa; Big Stone County, Minnesota; Cottonwood County, Minnesota; Faribault County, Minnesota; Freeborn County, Minnesota; Jackson County, Minnesota; Lyon County, Minnesota; Martin County, Minnesota; Murray County, Minnesota; Nobles County, Minnesota; Rock County, Minnesota; Yellow Medicine County, Minnesota; Brown County, South Dakota; Brule County, South Dakota; Buffalo County, South Dakota; Clay County, South Dakota; Day County, South Dakota; Deuel County, South Dakota; Gregory County, South Dakota; Lincoln County, South Dakota; Lyman County, South Dakota; Todd County, South Dakota; Tripp County, South Dakota; Turner County, South Dakota. Population to be Served: The target population includes all individuals living in the proposed service region. Underserved populations considered in project planning include Hispanic/Latino residents of the service area, Native American residents of the service area, and individuals with transportation barriers. Network Members: The Network consists of Sanford Health Network (non-profit healthcare system), Winner Regional Health (non-profit healthcare system), Ortonville Area Health Services (critical access hospital), and United Hospital District (critical access hospital). All Network members are physically located in a HRSA-designated rural area. All Network members are committed to creating a strategic bed-leveling initiative and in-person rural conference to strengthen the rural healthcare system in the upper Midwest. Experience in Serving Rural Underserved Populations: The four Network organizations have experience providing health care and services to rural residents in the proposed service area and are prepared to leverage this experience and knowledge for the proposed project. This project will bring greater access to local healthcare for rural residents, particularly underserved populations with barriers to care such as transportation.
