Focus Area(s): Schneck Medical Center’s focus area is care coordination in the rural setting. Proposed Service Area Indiana, Seymour and Surrounding Areas, Jackson County Abstract: Nearly 44% of the youth population are living in poverty and 31% of the adult population are living below 200% of Federal Poverty Level (FPL), with the median household income of the community ($59,504). Specifically, high rates of chronic conditions are plaguing the community, predisposing populations in a myriad of health conditions and poor health outcomes5: - 9% of adults age 20+ have diagnosed diabetes; - 29.4% of Medicare beneficiaries are living with heart disease; - 177.7 per 100,000 is the rate of age-adjusted death for cancer; - High rates of poisoning (including drug overdose) of 41.4 per 100,000 population - Age-adjusted death rate for stroke is 48.4 per 100,000; and - Age-adjusted death rate for unintentional injury is reported at 77.5 per 100,000 population. #1: Schneck Medical Center, Rural Hospital Schneck Medical Center is a leading medical provider in the Jackson County community, providing emergent medical care services and offering perinatal navigation through its new programming focus on health outcomes for rural mothers. Schneck Medical Center is located in the rural setting and is eager to implement project activities, identifying partners who are also ready to implement program activities, and will provide oversight of clinical programming to enhance the delivery of healthcare in the rural setting. #2: Nurse Family Partnership Nurse Family Partnership will be opening a site in Jackson County in 2024. Currently situated in a neighboring rural county that is serving Jackson County, NFP is eager to partner with Rural Network Programming to grow its footprint in the rural setting, enhance the perinatal navigation programming, and improve overall health outcomes for the rural mothers, infants, and their families. #3: Jackson County Health Department Situated in Jackson County and coordinating community programming, the Health Department has the resources and connections to advance the Network as a whole, supporting an improvement in health education and resources to the community to reduce health inequities among underserved and minority populations, a goal of the Department as a whole. Experience in Serving Rural Underserved Populations: SMC is licensed for nearly 166 beds, serving as a trusted medical provider for those seeking inpatient, emergent, delivery, and other specialty medical care services. SMC offers obstetrics services, a limited resource as Indiana’s birthing hospital totals continue to decline and offers general and specialty gynecology services to all residents in the Jackson County and surrounding communities. Through a focus on addressing maternal health and infant health outcomes, in direct response to Indiana’s staggering infant mortality rates of nearly 6.75 deaths for every 1,000 live births, the 10th highest infant mortality rate in the Country. SMC has taken this call to action in its daily mission to improve the health of Jackson and surrounding counties by continuing to identify opportunity to expand partnerships to address these disparities.
