Focus Area: Golden Valley Memorial Hospital’s focus area for this proposal will be developing an actionable plan to enhance care coordination among rural service providers as a means to improve health outcomes and address social determinants of health. Proposed Service Region: Henry County, St. Clair County and Benton County in Western Missouri. Project Summary: The Western Missouri Community Health Enhancement Network (WMCHEN) will seek to improve health outcomes for rural and historically underserved communities in Henry, Benton, and St. Clair Counties. All three counties in the proposed service area have been designated as rural by the Health Resources and Services Administration (HRSA) and St. Clair County has previously been designated by the Economic Development Administration as an area of persistent poverty. WMCHEN is intended to address critical healthcare issues facing the population in this area, including chronic disease management, substance use disorder, and behavioral health. The proposed network will consist of Golden Valley Memorial Hospital (GVMH), the region’s primary hospital and healthcare provider, Henry County Health Center, the public health authority for Henry County, Benton County Health Department, the public health authority for Benton County, St. Clair County Health Center, the public health authority for St. Clair County, and Compass Health Network, a local Federally Qualified Health Center. These organizations, all of which are located in HRSA-designated rural areas, have extensive experience partnering on critical healthcare programs and sharing resources, including through service referrals, information sharing, and the development of successful proposals to coordinate patient care. These organizations are now prepared to apply this preexisting infrastructure to WMCHEN. Under this proposal, GVMH, the Henry County Health Center, the Benton County Health Department, the St. Clair County Health Center, and Compass Health Network will formalize a network intended to address social determinants of health (SDoH) for individuals residing in the proposed service area. Critically, this proposal will leverage existing resources, including a common Community Information Exchange (CIE) platform capable of conducting efficient screenings and referrals based on an individual’s care needs. This approach will ultimately enable service providers to assess an individual’s needs holistically, make referrals to other relevant organizations based on the individual’s health and social wellbeing, and track outcomes in real time. Throughout the planning process, Network partners will ensure full participation by rural underserved populations through surveys, focus groups, and roundtables. GVMH was made aware of the HRSA Rural Health Network Development Planning Program through a technical assistance provider. GVMH is located in a Health Professional Shortage Area (HPSA) and is therefore eligible for a funding preference under this program, as authorized by U.S.C. 254c(h)(3).
