Network Planning Project Network Name: Crawford County Rural Health Planning Project Goal: To build a comprehensive network of high-quality healthcare services, increase access to care, and improve health outcomes for rural residents of Crawford County, Ohio. Legislative Aim: This project will work toward Legislative Aim # 3: Strengthen the rural health care system as a whole. Focus Area: The focus areas for this project are system coordination, increasing access to care, and improving health outcomes. Proposed Service Region: Crawford County, Ohio ABSTRACT BODY CONTENT Population to be Served The target population for this project is rural residents of Crawford County, Ohio with low household income who lack access to healthcare and prevention services. This includes people with low income who are under-served by the present system, including: ? People with chronic health conditions and disabilities; ? People with co-occurring physical and behavioral health disorders; ? Older adults; ? People with transportation barriers. Network Members • Third Street Family Health Services – Federally Qualified Health Center system. • Crawford County Public Health (prevention, reproductive health, sexual health services) • Crawford-Marion ADAMH Board (mental health and recovery planning and funding) • Community Counseling & Wellness Centers (behavioral healthcare services) • Avita Health Systems (emergency medicine, inpatient health, specialty care) Experience in Serving Rural Underserved Populations Third Street Family Health Services, lead applicant for this project, is a Federally Qualified Health Center that provides primary care, obstetrics and gynecology, oral health, behavioral health, podiatry, and community outreach in north central Ohio. Third Street serves rural populations in Crawford, Ashland, and Richland counties of Ohio, and has a location in Bucyrus, Ohio, that provides primary care and prevention services. Third Street works closely with healthcare and human service organizations in rural areas. Crawford County Public Health is a network partner with decades of experience in rural health needs assessment, planning, prevention, and direct service. Crawford-Marion ADAMH Board is a network partner with decades of experience in planning, funding, and evaluating behavioral healthcare services for rural populations. Two other partners bring extensive direct service experience with rural residents to the table. Avita Health Systems is the largest healthcare provider in the county with two general hospitals with emergency departments, a network of specialty care providers, and a wide range of other services. Community Counseling and Wellness Centers provides behavioral healthcare services, including mental health and recovery services, crisis intervention, school-based services, and prevention. Involving Rural Residents: This project is designed to improve health awareness, increase access to healthcare and specialty services, and improve health outcomes for rural residents who are underserved by the current system. Residents will participate in planning meetings, provide feedback through surveys and interviews, and help develop strategies for the planning project moving forward.
