All designated Rural by HRSA Population to be Served Any residents aged 60 and older in Delaware, Otsego and Schoharie Counties Network Members: Bassett Healthcare Network, hospital and community clinic system; Helios Care, in-home hospice and palliative care; At Home Care, licensed Home Health agency; Otsego County Office for the Aging, a County government department; Schoharie County Office for the Aging, a County government department; Leatherstocking Collaborative Health Partners, a network focused on health systems management and general public health issues Creating an Aging in Place continuum of care is a high priority for each agency; integration of services are necessary for a sustainable future and to achieve agency missions. Experience in Serving Rural Underserved Populations All six founding member agencies and the applicant agencies serve only rural residents. The largest healthcare provider regionally, the only hospice providing in-home palliative care; a home health agency that has thrived in a rural setting while others have not; two county agencies of rural governments dedicated to serving older rural adults and a collaborative network that focuses on solution to emerging health management issues in rural areas are the founders of the proposed network. Between them, there is more than 215 years of experience working in this rural setting and adapting state-of-the-art services to a rural delivery model.
