Focus Area: Share Health Southeast Georgia’s focus area is women’s primary care, wellness and prevention strategies. Proposed Service Region: Evans County and Tattnall County, Georgia. Both counties are fully rural. Abstract Body Content: The population to be served is rural, uninsured and underinsured lower-income women living in Evans and Tattnall County, Georgia. Evans and Tattnall Counties are both Health Professional Shortage Areas (HPSA). The combined population of the two counties is 33,616 with females comprising slightly less than half of the population. The majority of residents are white (66.8% in Tattnall and 64.3% in Evans). African Americans comprise 31.9% of the population in Evans and 29.3% in Tattnall, while the Hispanic population is 12% in Evans and 12.7% in Tattnall. A large proportion of residents are uninsured, at 18.6% in Evans and 19.4% in Tattnall, higher than the Georgia uninsured rate of 14.7%. There are numerous gaps in the health care system. There is a critical shortage of health providers. Neither county has a birthing hospital and no OB/GYNs have offices in these two counties. A Community Health Needs Assessment published in 2022 by Evans Memorial Hospital indicated a significant need for women’s health services and specialists in the community. Underserved populations, including African Americans and Latinos, have historically faced numerous health inequities resulting in poorer health outcomes. In addition to poverty and rurality, Latinos face unique challenges in accessing health care. Network Members: Share Health Southeast Georgia – Community-based nonprofit and lead applicant East Georgia Healthcare Center – Federally Qualified Communities Health Center Southeast Georgia Communities Project – Community based nonprofit Ware County Board of Health/Southeast Health District – public health department Evans County Cooperative Extension – County extension office Evans Memorial Hospital – rural, nonprofit hospital All network members are located in HRSA designated rural areas. Members are willing to investigate ways to increase access to health care through integrating resources, such as co-location of staff, sharing space and enhancing collaboration between partners. Letters of commitment are included from all network partners. Experience in Serving Rural Underserved Populations All network partners have long histories of providing clinical and/or supportive services to rural residents of southeast Georgia. All 16 counties in the Southeast Health District’s catchment area (where all partners are located and provide services) are HRSA designated rural counties. Established in 2016, Share Health Southeast Georgia works collaboratively with local communities to develop locally driven initiatives and activities, implement evidence-based services, and increase their capacity to identify, administer and apply for funding to initiate or expand their health-related programs and services. This is accomplished by providing 1:1 technical assistance, training, sustainability and strategic planning, data management and evaluation and quality assurance assistance. Funding Preference: Share Health Southeast Georgia is requesting a funding preference under Qualification # 3: Focus on Primary Care, Wellness and Prevention Strategies.
