Focus Area: East Central Illinois Healthcare Consortium focus area for this planning grant Goal 1: Incorporate three new member organization into Consortium Goal 2: Create strategies to reach out to the target populations that include those with SUD/OUD disorders and to those that are underserved. Proposed Service Region: Ford, Iroquois and Livingston counties Population to be served: 76,865 individuals living with in 22 zip codes that encompass Ford, Iroquois, and Livingston Counties. Capacity to Serve Rural Underserved Populations -The Consortium meets routinely to ensure that the grants are being administered in the manner identified n the Work Plan -The management team has been working with the Consortium since 2014 and has developed an informal network of organizations within the target service areas that are accustomed to working with rural clients and have vested interest in accessible healthcare The applicant is an experienced critical access hospital and has been serving rural populations for over 70 years.
