Network/Consortium Partnerships • Partners include: Valor Health, FMRI, and Healthwest. Additional partners to be engaged include the University of Utah and the Idaho Department of Health and Welfare. Experience in Serving Rural Underserved Populations • C-WHO is a non-profit with extensive experience serving rural communities in states across the country with an emphasis on provider training and behavioral health. Valor Health is a CAH and RHC with a long history of caring for patients in Gem County. Both Healthwest and FMRI are rural training sites for family medicine residencies in Idaho. Funding Preferences • C-WHO requests a funding preference based on Qualification 3: Focus on Primary Care, and Wellness and Prevention Strategies
