County: Okanogan; State: Washington ABSTRACT BODY CONTENT The overarching goal of the Methow Valley Community-Based Long-Term Care Network is to: Develop and implement the services, programs and workforce needed to support a sustainable community-based model of long-term care delivery that meets the immediate and future needs of our rural elderly. Specific objectives include: 1. Formalize a community-based network to support our aging population. 2. Develop and support a caregiving workforce. 3. Evaluate and recommend for implementation options for providing sufficient supported housing for seniors who cannot safely live at home. 4. Evaluate innovative strategies and supports to keep seniors in their homes. 5. Develop a strategic plan and business plan incorporating priorities, goals and objectives of the Network and addressing the planned implementation of services, programs and supports. Target Population: The two primary target populations in the Valley include working families and the aging population that today are struggling to stay in the Valley because of a lack of services, staff, and coordinated health care. The Network will have a specific focus on the lower income elderly that will require Medicaid support or on-going subsidy. The Network members also recognize that supports to working families, including elevating the caregiving role, increased compensation, and addressing social determinants of health must go “hand-in-hand” with designing the community-based long-term care system in the Valley. Methow at Home (MAT): Social Service Agency All three Network Members are located in a HRSA-designated rural area as defined by the Rural Health Grants Eligibility Analyzer. Experience in Serving Rural Underserved Populations: ? All of the Network Members are currently serving rural underserved populations in the Methow Valley. Jamie’s Place operates the only Adult Family Homes in the Valley. Family Health Centers provides comprehensive primary medical and dental care, orthodontic care, pharmacy, WIC, and integrated behavioral health. Methow at Home Provides volunteer services and supports, activities, and educational opportunities to support aging in place. ? All of the Network members were also members of the Senior Assessment for Support and Housing Committee (SASH) that conducted a Valley-wide senior services Needs Assessment in 2021. This assessment was in response to growing concerns about aging and supports in the Valley and led to the development of the Network and the Objectives and Strategies in the Network’s HRSA Network Planning Program grant project. Funding Preference: The Methow Valley Community-Based Long-Term Care Network is requesting a funding preference based on Qualification 1: Health Professional Shorage Area. The Methow Valley Service Area and the entirety of Okanogan County are in a designated Health Professional Shortage Area.
