Service Region: • Sullivan County, New York (HRSA rural) – see Attachment 2 for cities/towns. Target Population: The target population includes residents in Sullivan County who are experiencing SDOH that affect health care access, including access to affordable, reliable transportation, which is ranked as the top issue affecting health in Sullivan County, especially for those residents with special needs, per the CHNA. Network/Consortium Partnerships: Network partners include Garnet Health Medical Center – Catskills, Garnet Health Medical Center, Garnet Health Doctors, Sullivan County Division of Community Resources, and Rolling V Bus Corporation. Experience in Serving Underserved Rural Populations: The lead applicant, GHMC-C, is the primary clinical health care provider in Sullivan County. GHMC-C is designated by both the New York State Department of Health and the Centers for Medicare and Medicaid Services as a sole community provider and rural referral center for approximately 76,000 year-round residents of Sullivan County. The Board of Directors of Garnet Health Medical Center (GHMC) established a Community Health Department to address the significant health needs and disparities in its hospital catchment area. Board members understood that, as an anchor institution, it was critical for the hospital to play an important role in not just treating disease, but also preventing it and addressing the Social Determinants of Health (SDOH) impacting its community and patients. The Department, currently comprised of a Community Health Manager, is responsible for the state and federal hospital requirements that include completion of a community health needs assessment and community service plan, as well as regular tracking of the health system’s community benefit. The Department is currently (as of January 2022) hiring a two Community Health Coordinators. The Department reports to a health system Vice President of Strategic Planning and Marketing as well as a matrix report to GHMC-C’s CEO. GHMC Community Health staff play an active role in the community, dialoguing with outside organizations and stakeholders, and working with community partners to improve the health and health equity of the hospital catchment area through innovations in education, patient care, research, and community and physician engagement. Funding Preference: GHMC-C is requesting a funding preference based on qualifications 1 and 2. Sullivan County, NY, is in a designated HPSA and a MUA/MUP (see Attachment 8).
