Ryan White Title III HIV Capacity Development and Planning Grants - Wellness Equity Institute (WEI) proposes a comprehensive, multi-sector initiative to expand HIV prevention, treatment, and care coordination in Yucca Valley and Twentynine Palms, two rural communities in San Bernardino and Riverside Counties, California, that face significant healthcare disparities. By leveraging and strengthening strategic partnerships with managed care organizations, county health departments, street medicine providers, and social service agencies, WEI will enhance existing collaborations and establish new ones to create a sustainable, high-impact network for HIV care.
