Through this initiative, we aim to break down barriers to care for rural Mainers living with HIV/AIDS—many of whom face financial hardships, transportation challenges, and co-occurring chronic conditions that complicate their ability to stay engaged in care. Our project focuses on two key strategies: enhancing virtual visit capabilities and launching a Remote Patient Monitoring (RPM) program. By strengthening our partnership with the National Digital Equity Center, we will provide 11 tablets and hands-on digital literacy training to clients who lack access to technology, increasing virtual visit participation by at least 3%. Simultaneously, we will introduce RPM technology through Health Recovery Systems (HRS), equipping clients with tablets and linked monitoring devices—including a blood pressure cuff, oximeter, and scale—to track vital health data in real time. These tools will seamlessly transmit data to our clinical team, allowing for proactive intervention and direct video, audio, or text communication with patients. By integrating these telehealth solutions, the Horizon Program will improve access to care, reduce missed appointments, and better support clients in managing both HIV and co-occurring conditions. This project will enhance engagement in medical care, promote long-term health stability, and empower clients with the tools they need to stay connected to their care teams—no matter where they live.
