Ryan White Title III HIV Capacity Development and Planning Grants - Identification of Category: HIV Care Innovation: Inclusive care for underrepresented communities with disproportionately high rates of HIV Health Plus Indiana is a nonprofit AIDS service organization serving 17 counties across North Central and Southeastern Indiana. In order to strengthen organizational infrastructure and increase capacity for inclusive, HIV medical care in a rural community disproportionately impacted by HIV, mental health, and substance use disorders, Health Plus Indiana is applying for $150,000 in grant dollars from the Ryan White HIV/AIDS Program (RWHAP) Part C Capacity Development Program, which will fund the following programs and activities: Establish an onsite clinic to deliver supportive medical care, case management, and therapeutic measures Employ an Electronic Medical Record system to promote efficient monitoring and evaluation. Train and educate providers on trauma-informed, person-centered care These activities are united under the common goal of reducing barriers to care, such inaccessible care due to distance and provider shortage, continuity of care due to lack of transportation and poverty, and the underlying stigma associated with a HIV positive status in a rural community, in order to suppress the viral loads of Indiana’s Region 10 population.
