(AP, Inc.) is proposing a project in the HIV Care Innovation category, Inclusive Care for underrepresented communities with disproportionately high rates of HIV, Model of Care Infrastructure and Clinical Application activity. The proposed project will increase AP, Inc.’s capacity to deliver high-quality, effective, responsive and inclusive services to Black cis- and transgender women, ultimately increasing access to and retention in care services. Needs to be addressed: The program will address the high rates of HIV and other comorbidities in Dallas County. The priority population also experiences many barriers to accessing primary care, prevention services, and supportive services; barriers include poverty, stigma, a fear of being judged or misunderstood by medical providers, high rates of substance abuse and mental illness, poor health, and a lack of social support. Proposed activity: With the proposed funding, AP, Inc. will utilize the funding to support the costs of strategies to ensure inclusive care, which will include the incorporation of additional EMR modules to ensure the correct utilization of assessments and inclusive language and terminology, trainings for direct service staff, and cosmetic changes to AP, Inc.’s space to make it more welcoming and inclusive. Population group(s) to be served: The priority population that will be impacted by the proposed activity is transgender and gender non-conforming individuals, including Black and Latinx transgender women and other disproportionately impacted sub-populations in high-risk neighborhoods of Dallas County, TX. Among AP, Inc.’s clients, 75% are Black, 15% are Latinx, 5% are White, and 6% are classified as other, and 100% of clients live below the poverty line.
