Ryan White Title III HIV Capacity Development and Planning Grants - West Alabama AIDS Outreach, Inc. dba Five Horizons Health Services (FHHS) requests funding through the RWHAP Part C Capacity Development Program to expand access to behavioral health services across the entirety of our 53-county service area across Alabama and Mississippi. FHHS will accomplish this expansion through the implementation of a Collaborative Care Model (CCM). The Collaborative Care Model (CCM) is a comprehensive, integrated method of healthcare that involves a multi-disciplinary team of healthcare providers working together to deliver the highest quality of care to patients. CCM is particularly effective in managing chronic conditions, mental health issues, and other complex healthcare needs. As such, this model is extremely effective at promoting the successful implementation of an integrated behavioral health approach. The need for these services is well-documented. Many of the counties within our service area are listed as medically and economically underserved regions by HRSA. When a lack of accessible healthcare providers is combined with the rate of poverty, it is clear that many individuals in these communities are receiving little to no medical care with the inability to access care in the more populated areas. By implementing the CCM approach, not only will our providers address behavioral health needs, but these services will be done in collaboration with our medical and social services teams, which will result in an increase in client retention. We anticipate seeing an increase across the five stages of the HIV care continuum once this model is fully implemented across our service area.
