Organization History: Founded in 1981, CWWCHC handles over 28,000 patient encounters annually representing between 11,000 and 13,000 patients, with 87% being classified as a racial or ethnic minority, 64% living on incomes below 200% of the federal poverty level, 24.35% receiving coverage through Medicaid, 21.5% being homeless, and 45.33% being uninsured. Center services include primary/preventive medical and dental care, behavioral health, comprehensive HIV/STI treatment and care, 340B pharmacy, medical case management, case management, mobile medical and dental services, and community outreach. Needs to Be Addressed. CWWCHC’s service area has one of the highest HIV infection rates nationwide, with significant racial disparities in HIV prevalence. Unfortunately, CWWCHC is located in a designated Medically Underserved Area, a designated Dental Health Professional Shortage Area, and a Mecklenburg County Public Health Priority Area, where there are limited resources, especially for low income, uninsured and underinsured PLWH. Further, many of CWWCHC’s HIV-positive patients live close to or below the poverty level, and their only source of insurance is Medicaid. In addition to financial issues, PLWH throughout the service area face a host of other barriers to care, including unwillingness of some providers to treat HIV patients, social determinants of health factors, stigma, and discrimination. Summary of Proposed Project: As one of the only providers of comprehensive HIV care services in the region, CWWCHC has an urgent need to enhance its primary care services to better serve PLWH and respond to current unprecedented changes in local and regional healthcare landscapes. To increase access to care, CWWCHC will use RWHAP Part C Capacity Development Program funding to provide telehealth and remote patient monitoring (RPM) services. Telehealth services will be provided by the HIV Coordinator, the HIV Medical Case Manager, the SDOH Case Manager, the HIV Specialist Physician Assistant, a Licensed Clinical Social Worker, a Pharmacist, and the Dental Director. This cost effective, timely approach will optimize the provision of high quality HIV primary and support services. Anticipated outcomes include increased access to high quality primary care and support services, decreased health disparities, improved health status and quality of life, and enhanced organization capacity to reduce barriers to care for PLWH.
