Ryan White Title III HIV Capacity Development and Planning Grants - Project Abstract Summary for Optimus Health Care HIV Care Integration Initiative Organization: Optimus Health Care, Inc. Project Overview: Optimus Health Care aims to enhance the integration and coordination of HIV primary care with Behavioral Health, Oral Health, and Nutrition Counseling and Therapy using capacity development funds from the HIV Care Integration Initiative. This program is designed to improve access to comprehensive healthcare services for low-income, uninsured and underserved populations in Bridgeport, Connecticut, particularly those people living with HIV (PLWH). Background: Since the late 1980s, Optimus Health Care has been at the forefront of responding to the HIV/AIDS epidemic in Fairfield County, particularly in Bridgeport, Connecticut, through various services funded mainly by the Ryan White Program. Despite ongoing efforts, there remains to be a significant need for integrated care services among the HIV-positive population, many of whom suffer from co-morbidities such as mental health disorders and chronic diseases. Needs Assessment: Recent data highlights Bridgeport as having one of the highest HIV incidence rates in Connecticut. Structural inequalities significantly impact the prevalence and treatment outcomes of HIV among communities of color in the region. Less than 5% of program participants currently receive mental health services, although these are critically needed. Project Objectives: Care Coordination Enhancement: Utilize a newly built multidisciplinary conference room for the various disciplines (Primary Care, Behavioral Health, Oral Health, and Nutrition) to provide patient care and integration among various health services. Policy and Workflow Development: Establish effective workflows and policies for tracking, coordinating care integration, referrals, linkage to care, and retention. Service Expansion: Increase mental health service access, enhance oral health care capacity, and integrate nutrition counseling with HIV primary care. Stakeholder Engagement: Facilitate better collaboration between Behavioral Health professionals and HIV primary care providers to address the multifaceted needs of the HIV population. Expected Outcomes: Improved access to mental health and oral health services for people living with HIV. Increased percentage of patients receiving integrated nutrition counseling and HIV care. Enhanced patient outcomes through integrated service delivery and holistic care approaches. Reduction in new HIV infections through improved care coordination and community-based education. Conclusion: By addressing the complex needs of people living with HIV (PLWH) and tackling structural inequalities that exacerbate health disparities, Optimus Health Care's initiative strives to significantly improve the quality of life for affected individuals and reduce the overall incidence of HIV in the Bridgeport area.
