Ryan White Title III HIV Capacity Development and Planning Grants - Open Door Community Health Centers (ODCHC) provides comprehensive HIV Primary Medical Care services at three community health centers in Del Norte and Humboldt Counties, an isolated region on California’s northernmost coast. More than half of the regions’ 164,000 residents live outside incorporated areas, often isolated by distance and mountainous terrain, in an area covering over 7,250 square miles. The nearest metropolitan areas are Sacramento, CA (5 hours southeast), San Francisco, CA (6 hours south) and Portland, OR (8 hours north). While ODCHC serves a broad spectrum of people, the mission specifically targets low income and underserved community members. ODCHC is the only HIV primary care provider in the area, and our longtime provider David Horwitz has been providing care to PLWHA since the beginning of the epidemic having treated the first case of HIV in Humboldt County. ODCHC first received Ryan White Part C Outpatient EIS Program grant funding in 2002, and we pride ourselves on delivering high quality, compassionate care to our community. As a learning organization focused on improving our ability to make data-informed decisions we are keenly aware of the need to improve our infrastructure to facilitate inclusive care. We have opportunities all along the patients journey with us to make these improvements – from registration and intake to care plans and clinical support. The Inclusive Care for Better Health Outcomes Project aims to accomplish three primary goals: improve the rate and validity of Sexual Orientation and Gender Identity/Expression data, align health maintenance alerts in our electronic health records to be inclusive and comprehensive for all genders, and to develop a hormone specific injection training curriculum. All of these activities serve to not only improve individual patient health outcomes, but also to foster an inclusive and welcoming environment that supports patients of all identities confidently accessing care.
