The goal of the project will be to improve Continuum of Care outcomes for Underrepresented Communities with Disproportionately High Rates of HIV in the New Orleans area. The priority subpopulations are Black Cis and Transgender women, Transgender Latinx women, and Black Latinx individuals who have rates of viral suppression lower than the overall average for CrescentCare’s HIV patients. Focus groups will be held to learn more about causes of disparities for these individuals and to better understand their needs, and potential barriers to improved outcomes. The content from the focus groups will help inform didactic trainings that will be developed and provided to clinical and administrative staff on topics such as cultural competency, stigma, discrimination, and implicit bias, and to also address social determinants of health. New Orleans has one of the highest rates of individuals living with HIV as well as one of the highest rates of new diagnoses in the country. CrescentCare serves over 2,800 HIV patients per year and the majority have incomes below 100% of the federal poverty level and are Black and a growing number are Latinx. Black, Latinx, and transgender individuals are disproportionately represented in the number of individuals living with HIV and in the number of new HIV diagnoses. Individuals living with HIV in the New Orleans area face many social factors which can impact their health, including poverty, housing issues, low health literacy, food insecurity, high crime and violence rates, racism and discrimination, and the constant threat of natural disasters such as hurricanes. The impact of these factors can be seen in the overall viral suppression rate for all HIV patients at CrescentCare which is slightly lower than the national average with significantly lower rates for the subpopulations that will be the focus of this project. These trainings will enhance CrescentCare’s cultural responsiveness and improve health outcomes for those experiencing disparities. In addition, information from the focus groups will help CrescentCare to improve its understanding of how social determinants of health impact health care outcomes for these individuals and guide development of specific strategies to address those issues. While CrescentCare currently provides cultural humility training to all staff, this project will allow additional trainings to be developed and implemented that particularly address the needs of these underrepresented subpopulations. A total of $150,000 is requested for the one-year period. CrescentCare meets the requirement for funding preference for underserved areas as outlined in Attachment 2 of the application.
