Ryan White Title III HIV Capacity Development and Planning Grants - Cornell Scott Hill Health Corporation Ryan White C Capacity Building Program Oral Health Integration Initiative - The Cornell Scott Hill Health Corporation Oral Health Integration Project will increase the number of Persons Living with AIDS (PLWA) in the City of New Haven who receive oral health services from Cornell Scott Hill Health Corporation. Currently about 100 of our 300 Ryan White clients are provided with dental services at least once per year. Using the evidence based frameworks HRSA HAB Integration of Oral Health and Primary Care Technical Assistance Toolkit and the Safety Net Medical Home Initiative, Cornell Scott will train its Ryan White providers, case managers, and dental staff to better identify oral health issues within our clients and to address barriers to care as well. This will result in the Ryan White providers and case managers being trained in doing a cursory dental exam as well as case managers being trained in dental screens and case management to assist in patient navigation around insurance, transportation, and scheduling barriers. In addition, the program as part of the larger New Haven HIV community, will conduct extensive community outreach and education on the importance of oral health for PLWAs through a variety of soft and hard media, reaching 2,000 community members. As a result, we will increase the number of Ryan White patients who have a dental exam by 50% to 150; and the number of oral health patients with HIV who have a dental history, dental treatment plan developed, and periodontal screening completed.
