Ryan White Title III HIV Capacity Development and Planning Grants - Community Health Center, Inc. proposes to integrate existing successful infrastructure elements with an innovative new model of care which aims to bring medical, behavioral health, and support services to individuals living with or at risk for HIV. The proposed project falls under the category of HIV Care Innovation; Inclusive Care for underrepresented communities with disproportionately high rates of HIV. Leveraging an existing healthcare delivery model which includes advanced education for providers and our customized Mobile Health Unit (MHU), purchased in 2023, CHCI will engage diverse and underserved populations where they live, work, play and receive services. Expert HIV providers and clinical teams will provide monthly didactic and case presentation sessions focused on the MHU and clinical care designed to reach individuals living with or at high risk for HIV to clinical teams working in the MHU program with an emphasis on HIV prevention, diagnosis, treatment, and retention in care. The MHU team will provide primary care along with HIV/HCV specialty care, prevention education, STI screening, and other needed services from the flexible setting of the MHU. This innovative service delivery model addresses a need to provide access for hard to reach populations where they live, work, play, and receive services and eliminates common barriers to care such as transportation, childcare, stigma, and structural barriers. In keeping with the Ending the HIV Epidemic (EHE) Initiative, the program model will link people with HIV who are newly diagnosed, diagnosed but not currently in care, or diagnosed and in care but not virally suppressed to the essential HIV care, treatment, and support services needed to help them reach viral load suppression. The MHU model will deliver widespread and comprehensive HIV/STI prevention education, engagement in primary care and behavioral health services, access to oral healthcare, and other support services that directly respond to the gaps identified by the communities we serve and more specifically individuals living with HIV in the communities we serve. Funded work will build sustainable infrastructure for the MHU as an established model which will generate revenue from payers to fund the program for the long term.
