Funding amount requested: $150,000.00 Bond Community Health Center, Inc. (Bond), a federally qualified health center, has provided comprehensive, culturally, and linguistically appropriate primary health care to the uninsured and underinsured of Tallahassee, Leon County, Florida, and surrounding communities since 1984. Bond has provided uninterrupted outpatient HIV primary care services since 2001, and since 2012, as a Ryan White HIV/AIDS Program Part D provider for an eight-county Consortia Area 2B (Leon, Jefferson, Gadsden, Franklin, Liberty, Madison, Taylor, and Wakulla Counties) wherein outpatient HIV primary care services are targeted to low-income, vulnerable, medically underserved people living with HIV/AIDS (PLWHA). Six of these counties are included in HRSA’s Rural Health Grants Eligibility Analyzer; therefore, Bond is requesting funding preference. In addition to Women, Infant, Children and Youth (WICY) Part D patients, Bond serves HIV+ adult patients who would qualify for the RW Part C program. These patients have chosen Bond for their primary care due to long-term relationships and trust. Bond utilizes a patient and family-centered model of care, achieving Accreditation Association for Ambulatory Health Care accreditation and deemed a Patient-Centered Medical Home. Bond is applying for RW HIV/AIDS Program C Capacity Development Program in Category 2: Infrastructure Development, for the Emergency Preparedness Activity. Proposed Project: Bond will develop and execute an implementation and testing plan for its proposed Emergency Management and Continuity of Operations Plan, incorporating lessons learned from recent HRSA Ryan White HIV/AIDS Program Part D Supplemental Funding on decreasing stigma and discrimination. Needs to be Addressed: Bond utilizes the Kaiser Permanente Hazard and Vulnerability Assessment Tool which identified Tropical Storm/Hurricane Watch, and Tropical Storm/Hurricane Warning as two of the top three events with the highest risk for Bond’s patients and community. This project will support Bond’s needs in developing an implementation/ testing approach of proposed plans, taking input from multiple stakeholders. The team will purposefully focus efforts to support PLWHA, to ensure total number PLWHA in medical care and/or total virally suppressed, two measures included in the HIV Care Continuum, are not adversely affected. Proposed Services: The implementation/testing approach for Bond’s proposed Emergency Plan will support the transition to a combined plain language and color-coded emergency alert model, with staff trainings; development of pre-scripted patient messaging tied to emergency events, with focused messages for PLWHA,; creation of response procedures specific to Bond vehicle(s) used to transport patients, including PLWHA; development of a trending summary report related to drills and actual events; crafting of criteria for identifying and disseminating emergency preparedness learnings from local partners; and the development of a ‘mini-toolkit’ where instructions to trainings, documentation, and frequently-asked-questions, are stored and which could be expanded to enhance efforts to support additional populations, and/or be shared with others in the community. Target Populations: During recent years, the Bond Ryan White HIV/AIDS Program provided services to 220 patients in 2022 (HIV+ adults and Part D); and 297 patients in 2023. While this project includes a purposeful focus on PLWHA, Bond’s entire patient population will benefit.
