Ryan White Title III HIV Capacity Development and Planning Grants - The Board of Regents, NSHE obo the University of Nevada, Las Vegas (UNLV) is pleased to apply for the FY 2023 RWHAP Part C Capacity Development Program Funding for the amount of $150,000. This funding would support us in providing an HIV Care Innovation program on Gender-Affirming care, Gender Affirming Medical Treatment (Didactic Training and Education). Kirk Kerkorian School of Medicine at UNLV aspires to achieve diversity among our students, faculty, staff, and leadership that is reflective of Southern Nevada and we celebrate a culture of inclusion where all members of our medical community are welcomed, respected, and heard. We honor those devoted to increasing health equity and providing care to all in the context of cultural humility. As an academic medical center, it is our privilege and responsibility to train and mentor future healthcare providers who will understand, appreciate, and advocate for diversity, equity, and inclusion and are well-positioned to provide the best care to all people. https://www.unlv.edu/medicine/diversity Transgender and gender diverse people face structural barriers in accessing healthcare, mental health, and economic resources, in Clark County, Nevada and worldwide. They are more likely to attempt suicide, experience homelessness, suffer from mental health issues with the highest rates of tobacco, drug, and alcohol use. Yet they also are less likely than cisgender individuals to seek treatment - over 20% reporting they avoid healthcare providers due to fear of discrimination. The Kirk Kerkorian School of Medicine at UNLV is working towards establishing the Nevada Gender Affirming Healthcare Program (NGHAP), a collaboration between the faculty practice plan’s (UNLV Health), primary care and multiple specialties, and the community. In the past decades, it has been increasingly recognized that transgender and gender diverse people can be well-served in the primary care setting. With additional training, primary care providers can provide gender-affirming services in addition to life-long care and monitoring, while working closely with specialists in endocrinology, psychiatry, surgery and HIV care. Additional training is also needed in surgical care to allow experienced surgeons to observe and get hands-on experience in gender-affirming procedures. The Kirk Kerkorian School of Medicine at UNLV is uniquely positioned to collaborate with academia, clinical delivery sites, and the community to increase life-saving access to care to the transgender community. The first step in this endeavor is to create a comprehensive didactic and hands on curriculum for residents and medical students to enable them to provide gender affirming care for transgender and gender diverse persons inclusive of HIV care and prevention. The school of medicine will also work to partner with Nevada’s Project ECHO, Nevada AIDS Education Training Center Program, and UNLV School of Nursing to provide continuing education credit training via in-person and remote learning options for all medical professionals in Nevada, regardless of location. By creating this 2 pronged approach, we can provide much needed training to current medical professionals while delivering an evidence-based, innovative and inclusive education for future medical professional. Approximately 2% of the people living in Nevada with HIV are transgender or gender diverse. Providing training and education specific to this population, including but not limited to medical, social, legal, and psychological needs can help reduce stigma, prioritize health equity, reduce health disparities and improve health outcomes for both transgender/gender diverse and HIV populations.
